About the Role
In this role you will provide leadership, guidance and support to our Community Care Workers who provide services to the assessed needs of frail aged older people, people with dementia, people with disabilities are met in a way that preserves or enhances their wellbeing.

This is a temporary full-time opportunity on a 6 months contract.

Key Responsibilities:

  • Provide provision, co-ordination, development and evaluation of Community Care Services
  • Ensure the delivery of high quality, cost effective Community Care Services.
  • Response to daily contact from clients regarding service delivery arrangements
  • Identify clients in need of reassessment or who may require care management
  • Promote teamwork and foster exchange of ideas
  • Ensure that all staff are appropriately skilled and developed to provide a quality Community Care Service and maintain high standards of service provision through appropriate staff practices and a clear identification of duties.

About you  
To be successful in this role, you will have experience in a leadership role providing support to frail aged older people, people with dementia, people with disabilities and their carers. You will have experience leading and motivating a team, service administration, rostering and team development.

  • Ability to communicate empathetically and effectively with older people, people with dementia, people with disabilities and their carers
  • Knowledge and awareness of issues relating to frail aged older people, people with dementia, people with disabilities and their carers
  • Understanding and ability to implement personnel practices, including those related to Occupational Health & Safety and Equal Employment Opportunity legislation and policies and employees training and development
  • Highly developed written, verbal, negotiation and conflict resolution skills
  • Post-secondary training/qualifications in a human services field
  • A current Victoria Drivers Licence
  • Covid19 vaccinations required for this role.
