Job Description:
The RN Case Manager provide a collaborative process of assessment, planning, facilitation, case management, evaluation and advocacy for options and services to meet each patient’s needs to achieve optimal clinical, financial, and satisfactory patient outcomes. The overall goal is to enhance the quality of patient care throughout the continuum of care to achieve excellence in healthcare delivery, within a framework of fiscal responsibility and focus on optimization of the transition between the outpatient, inpatient, and post-acute settings
Minimum Qualifications:
1. BSN with current licensure recognized by the State of Ohio is required.
2. Minimum of 3 years clinical experience required – preferably in specialty field.
3. Possess organizational, problem-solving, and critical thinking skills.
4. Demonstrated ability to work independently and effectively under minimal supervision.
5. Excellent written, verbal and collaborative group communication skills.
6. BLS Certification is required.
7. Valid driver’s license and must maintain state minimum auto liability coverage.
