We are seeking an experienced case manager with a clinical/health background to join our Team at KinCare, based in our Canberra Office.

This is a rewarding role, where you will engage with elderly customers living in the community to support their health and wellbeing, with the purpose of empowering our customers to remain living independently in the comfort of their own home.

Responsibilities include:

  • Conducting needs assessments and identifying personal-centered solutions and plans to enable our Customers to achieve their goals and enhance their quality of life
  • Working closely with the ACT and wider NSW team to effectively case manage the delivery of KinCare services, ensuring ultimate customer satisfaction
  • Managing individual customer budgets to enable choice and empowerment
  • Building and maintaining industry relationships to grow KinCare as a preferred provider of in-home aged care services

To be successful in the role, you will need to have a nursing or allied health background to draw upon and strong interpersonal and client-management skills.

Ideally, you will have previous case management experience and knowledge of the community/aged care system and Home Care Packages (HCP) is desirable.
